I understand and acknowledge that I am fully aware of, and voluntarily assume, all risks (including but not limited to the risk of serious bodily injury, death, property damage, or loss) associated with: 1. My participation in any training activities conducted by or affiliated with Rev-Tac Firearm Instruction L.L.C. (“Rev-Tac”); 2. My use of any firearms, ammunition, equipment, or facilities provided by Rev-Tac; and 3. My presence on or use of any shooting ranges, training areas, or properties owned, operated, maintained, leased, or used by Rev-Tac, including but not limited to: —All property owned by Verzani Farms, including:  Location #201 – Latitude 42.52575, Longitude -96.64032 / Latitude 42.52282, Longitude -96.63457 (Ponca Township, NE 68770)  Location #301 – Latitude 42.56388, Longitude -96.68061 (Ponca Township, NE 68770)  Location #401 – Latitude 42.5445, Longitude -96.67887 (87783 593 Ave., Ponca, NE 68770) —Any and all property or ranges owned or managed by Kyle Crosgrove. I acknowledge that Rev-Tac provides firearms, ammunition, and equipment “as is,” and disclaims all warranties, express or implied, including any warranty of merchantability or fitness for a particular purpose. Knowing and understanding the risks described above, I agree—on behalf of myself, my heirs, executors, administrators, and assigns—to assume full responsibility for any and all injuries, losses, or damages that may occur to me or my property. I hereby release, hold harmless, and agree to indemnify and defend Rev-Tac, Verzani Farms, Kyle Crosgrove, and their respective owners, officers, directors, agents, instructors, staff, volunteers, employees, successors, and assigns from and against any and all claims, liabilities, damages, or causes of action arising from or related to my participation, presence, or use of the facilities, property, firearms, ammunition, or equipment, regardless of cause and including any claim based on negligence. Use of Sig Sauer P320 and Similar Firearms: Participants who bring and/or use a Sig Sauer P320, including any of its variants do so at their own risk. Due to reported incidents involving unintentional discharges with these models, all users are solely responsible for the safe handling and operation of their firearm. In the event of a negligent discharge—defined as any unintentional firing of the weapon—resulting in injury to any person or damage to property, the participant assumes full responsibility and liability. The range, its owners, staff, and affiliates are not liable for any consequences arising from the use of such firearms. I certify that I am at least 18 years of age and that I have carefully read this Assumption of Risk and General Release Form. I understand and agree that no oral or written representations can or will alter the contents of this document. MINOR PARTICIPANT (UNDER 18) – PARENT/GUARDIAN CONSENT & RELEASE I, the undersigned, am the parent or legal guardian of the minor named below. I have read and fully understand the terms of this Rev-Tac Liability Waiver. I give my permission for the minor to participate in all training activities, range activities, and the use of firearms, ammunition, equipment, and facilities operated, owned, or used by Revolution Tactical L.L.C. (“Rev-Tac”), Verzani Farms, and Kyle Crosgrove, as outlined above. I acknowledge and accept all risks on behalf of the minor, and I agree to be bound by all terms of this waiver as if I were the participant. I voluntarily release, hold harmless, and agree to indemnify Rev-Tac, Verzani Farms, Kyle Crosgrove, and all affiliated parties from any and all claims or liabilities arising out of or related to the minor’s participation or presence.
